Good afternoon. Student Support and Health Services will be holding an immunization clinic on May 2, 2017 for EGUSD students in grades Kinder-12. See attached flyers in English, Spanish, Vietnamese, Chinese and Hmong. Families must meet one of the eligibility requirements as stated and a legal guardian must be present. Clinic will be held at the Robert L. Trigg Education Center. Please share this information with eligible families that are in need of immunizations for their children. PLEASE NOTE-Families need to bring a copy of their child’s shot records or the immunization print out from the school. Students, please click the following link and complete the registration form. An e-mail confirmation will be sent to you within 24 hours of submission. Print out the form, sign the form and turn it into the counseling office. Forms can be printed from the Career Center or Library. Students will be enrolled based on their enrollment priority. Please read all information before submitting your form.
